Why has a holistic arthritis pain management program become so crucial?

This indeed is a worrisome trend.

American Geriatrics Society and Osteoarthritis Pain

A new 2009 version of the American Geriatrics Society clinical practice guideline for the pharmacological management of pain in older persons has been issued. The new guideline recommends that acetaminophen (like Tylenol) be considered as initial and ongoing drug therapy of patients with mild to moderate musculoskeletal pain, such as the chronic pain of osteoarthritis.

The big change from the 2002 guideline is the recommendation that doctors rely less on NSAIDs (like aspirin) and COX-2 inhibitors (like Celebrex) for the management of pain in elderly people, saying, "non selective NSAIDs and COX-2 selective inhibitors be considered rarely, with caution, in highly selected individuals." They recognize that these drug have too many side-effects. It is gratifying to see a mainstream medical group acknowledge that at least one form of chemical painkiller is not worth the safety risk.

Unfortunately, they go on to recommend that "all patients with moderate to severe pain, pain-related functional impairment or diminished quality of life due to pain be considered for opioid therapy" (like Methadone). They also provide new references and discussions regarding use and limitations of other drugs for pain problems.

An interesting departure is that they no longer recommend corticosteroids for the control of osteoarthritis, stating that osteoarthritis should not be considered an inflammatory disorder.

Recognizing that many people need something other than pharmacological (drug) therapy they indicate that alternatives exist, which they should discuss with their physician.

We welcome every move away from invasive, side-effect laden drugs and look forward to the day when effective and safe natural pain management methods become more accepted, even recommended.
